6/23/2023 EnchantaVaughn Approved 1.Behavior: Upon QMHP-Câs arrival, Ashiyah showed a lack of empathy and consideration for the session. Ashiyah displayed a poor attitude toward mental health professional due to personal conflicts and external distractions. 2. Behavior: Ashiyah began to display rebellious and stubborn moods, as she then showed disrespect and negative behaviors towards QMHP-C. Ashiyah was disrespectful refusing to process her emotions by walking off and exhibiting increased frustration. 2. Behavior: Ashiyah contributed to the session and did not need any redirection throughout the session as she participated fully in session. Ashiyah reported to QMHP-C on how she continues to try to communicate with her mother but she does not connect with her ( poor parent-child relationship concerns ) all the time and she thinks it stems from her sisterâs father because the two of them always arguing about him. 1.Intervention: QMHP-C assessed her behaviors and mood in a genuine state when she was naturally being herself. QMHP-C inquired about Ashiyahâs day and provided affirmations. QMHP-C probed about any angry feelings or the inclination of safe and unsafe behaviors. Ashiyah modeled different ways to walk away from a conflict when frustrated. Ashiyah modeled how to separate herself from negative situations by counting to 10. Ashiyah demonstrated how to verbalize her feelings without yelling and being aggressive. 2. Intervention: certain behaviors and how she sometimes feels like the only way to respond is in a negative manner. QMHP-C continued to engage with Ashiyah and provided positive reinforcement as she participated in the session. QMHP-C educated Ashiyah about using good manners at home and offering alternative methods and suggestions to model appropriate language that Ashiyah should use to ask for things or to get someoneâs attention. 3. Intervention: QMHP-C provided Ashiyah with examples of how to engage her mother in conversation and discussed being receptive to criticism and feedback from her mother whenever they engage in conversation with each other. QMHP-C role-played with Ashiyah reactive and nonreactive responses as well as respecting her mother's limitations and boundaries which can be perceived as disrespectful. QMHP-C discussed her ability to follow rules and regulations set by her mother as she has been refusing to listen to directives. 1.Response: During each intervention, mother expressed Ashiyahâs behaviors have improved but she still has to remind Ashiyah about remaining consistent with reacting positively to negative situations. Mother and Ashiyah displayed good communication with each other. Although mom is more stressed with Ashiyah choices and behaviors in the home despite challenges, she explains that she wants to spend more time with her daughter each day to teach her the principle of survival and gratitude. Ashiyah responds well to praise and encouragement and is seen paying attention to the activity and listening to QMHP-C 2.Response: Ashiyah expressed positive thoughts about her strengths but seem to be struggling with her weaknesses. Mother states to QMHP-C that she has considered implementing a minor schedule for Ashiyah to assist her with her completion of chores in the home, including but not limited to her bedroom. Mother mentions that schedule assists her as well and allows her to remain focused on completing individual tasks without negative reactions. 3. Response: Ashiyah reports that she has close relational ties with her mother sometimes which depends on their week. Through role-playing Ashiyah was able to understand reactive and non-reactive responses and learning that all feedback from her mother maybe positive/negative and that she has to take in consideration that she has to take the good with the bad.
